Insurance and Forms
Our staff will work with you to help process and file your insurance claims. We accept most major insurance firms including: Aetna, Anthem/Blue Cross, Cigna, EyeMed, Indiana University Health, Medicare, Sagamore, United Health, Vision Care, and Vision Service Plan (VSP) insurance plans.
Out of network?
Patients are usually able to go out of network with any other insurance plan and file an itemized bill for products and services received at our office. The Eye Doctors will provide a complete, itemized statement for you to file directly with your insurance company if you choose to use the out-of-network feature of your benefit plan. Call our office for further details, as these plans do vary.
